"Advance care planning begins by thinking about your beliefs, values and wishes regarding future health care treatment, and talking about them with family or friends, and health care provider(s). When the people you trust know what is important to you with regard to future health care treatment, it is easier for them to make decisions on your behalf" (BC government site - Advance Care Planning, January 14, 2024). The provincial government in partnership with the BC health authorities of have created My Voice that contains information and blank, fillable documents that can act as templates for advance care planning (For more language options please see the BC government website).
