Making Future Financial and Health Care Decisions or an Advance Care Plan (ACP), is a written summary of an adult's wishes or instructions to guide a substitute decision maker. A substitute decision maker may be asked by a physician or other health care provider to make a health care treatment decision on behalf of the adult. Our ACP session includes assistance and advice on creating a Representation Agreements and Enduring Power of Attorney. These services are integrated with our counselling services and are part of our holistic approach to treatment.
